CLIENT RIGHTS

 

Welcome to our Client Rights page.  Your rights as a client are guarantees of such aspects of care as being treated with consideration and respect for personal dignity, autonomy and privacy and being able to make informed choices of the services provided.  At the time of your first appointment, you will receive a list of your rights.  Please read the list carefully and talk to your counselor/worker if you have any questions. Family Service also has a Client Rights Officer who is available to answer your questions.  Her name is Anita Swift, and you can reach her at 513-354-5633. 

 Family Service supports the right of any current or former client of the agency to have his/her grievances or complaints heard in a fair and responsive way. If you believe you have been discriminated against or wish to express a complaint or grievance, please follow the steps below:

 1)      Talk with your counselor/worker directly. (You may omit this step if this feels too uncomfortable for you.)

2)      Talk with the supervisor of your counselor/worker. (You may also omit this step if you prefer.)

3)      Talk with the Clients Rights Officer—Anita Swift at 513-354-5633.

 At any stage, the staff member will attempt to resolve the concern or issue you have. If these steps have not been helpful, the Client Rights Officer (CRO) will help you resolve the issue or assist you in filing a formal grievance, following the Grievance Procedure included with the documents you received at your first appointment. If you lose these pages, you may request a copy from your counselor/worker, a center secretary, or another staff person.

 The Client Rights Officer must assist you in filing a grievance upon request and must respond promptly by contacting you within 24 hours. You should give the grievance in writing or orally to the CRO, or in the event of the absence of the CRO, to the President of Family Service or for Kentucky clients, to the Executive Director of Northern Kentucky. Grievances must be in writing and signed by you or by the individual filing the grievance on your behalf.
